2003-2006 Mazda6 Fan Control Module Special Service Program (SSP) 71
Mazda Motor Corporation has decided to conduct a Special Service Program (SSP) to replace the Fan Control Module on certain 2003-2006 Mazda6 vehicles produced from August 5, 2002 through November 7, 2005.
On certain Mazda6 vehicles, it is possible that a crack may occur in the soldered part of the IC terminal in the fan control module, causing poor continuity. Under this condition, the cooling fan may stop operating, causing the coolant temperature to rise or a fan noise from the module.
Owners of subject vehicles will be notified by first class mail beginning April 9, 2007.

CONDITION OF CONCERN
On certain Mazda6 vehicles, it is possible that a crack may occur in the soldered part of the IC terminal in the fan control module, causing poor continuity. Under this condition, the cooling fan may stop operating, causing the coolant temperature to rise, or a fan noise from the module.

RENTAL CAR
A rental car may be provided to the customer if eligible based on the terms and conditions of the Rental Car Reimbursement Program, policy 12.0. Rental car reimbursements are available only on 2001 and newer vehicles within the mileage and time limitations under the New Vehicle Limited Warranty. If the customer was placed in a rental car while the campaign was being completed, submit a separate claim/problem using the standard rental claim information.

ATTACHMENT II REPAIR PROCEDURE
A. VEHICLE INSPECTION PROCEDURE
- Verify that the vehicle is within one of the following ranges:
- 2003 - 2006 Mazda6 3.0L vehicles built from August 5, 2002 through November 7, 2005

- If the vehicle is within any of the above ranges, proceed to Step 2.
- If the vehicle is not within any of the above ranges, return the vehicle to the customer or inventory.

B. INSPECTION PROCEDURE
Review the warranty record of the subject vehicle for the previous replacement of the fan control module.
- If the fan control module was previously replaced with part number AJ51-15-15YA or AJY2-15-SC0, exclude the vehicle from the campaign. Proceed directly to "E. CAMPAIGN LABEL INSTALLATION" to complete and affix a campaign label to the vehicle's bulkhead. Return the vehicle to dealer inventory or to the customer.
- If the fan control module was not previously replaced, proceed to "C. REPAIR PROCEDURE" and perform the repair.
C. REPAIR PROCEDURE
- Record customer's pre-set radio stations.
- Disconnect negative battery cable.
- Remove air cleaner box.
- Remove two (2) Fan Control Module bolts; discard bolts.
- Disconnect three (3) electrical connectors from Fan Control Module.
- Attach three (3) connectors to new Fan Control Module.
- Secure module with two (2) new bolts provided in kit.
Tightening Torque: 26-34 in-lbf (29-40 kgf-cm)
- Reinstall air cleaner box.
- Reconnect negative battery cable.
- Re-enter customer's radio pre-set stations.
- (2006 MODEL ONLY): Perform "D. Power Window Initialization Procedure".
D. POWER WINDOW INITIALIZATION PROCEDURE (2006 MODEL ONLY)
- Turn ignition to ON position.
- From driver's seat ensure window lockout switch is NOT depressed.
- Press window switch down to fully open door window.
- Pull window switch up to fully close door window; hold switch up for two (2) seconds; release switch.
- Move to front passenger seat and repeat Steps 3 - 4 on passenger window switch.
- Verify proper window operation using each door switch.
- Proceed to "E. CAMPAIGN LABEL INSTALLATION".
E. CAMPAIGN LABEL INSTALLATION
Complete a "Campaign Label" with the SSP71 written on the sticker and affix it to the vehicle's bulkhead. Refer to illustration under "A. VEHICLE INSPECTION PROCEDURE".
REIMBURSEMENT PLAN
Requirements for Reimbursement
If you meet all of the following requirements, you are eligible to receive reimbursement under this plan:
- An authorized Mazda dealer has inspected your vehicle and completed the 2003-2006 Mazda6 Fan Control Module - Special Service Program (SSP) 71.
- You own or have owned a subject 2003-2006 Mazda6 within the VIN range:VIN RANGE REFERENCE CHART

Procedure for Reimbursement Request
Once your vehicle has been inspected, repaired or the Fan Control Module replaced by an authorized Mazda dealer, you may apply for reimbursement by doing the following:
- Complete the Reimbursement Application Form found on the reverse side of this page.
- Mail the Reimbursement Application Form with a legible copy of the paid repair order and/or invoice using the enclosed envelope.
- Retain copies of the paid repair order or invoice and this application form for your records.
If you wish to correspond with Mazda regarding this reimbursement plan, please write to the above address and refer to your vehicle identification number (VIN).
Any reimbursement application form that is incomplete, illegible, or sent without the legible copy of the paid repair order or invoice will be returned for completion. If Mazda has any questions concerning your application for reimbursement, you may be contacted. Please allow 6-8 weeks for processing.

Dear Mazda Owner:
Mazda Motor Corporation has decided to conduct a Special Service Program to replace the Fan Control Module on certain 2003-2006 Mazda6 vehicles produced from August 5, 2002 through November 7, 2005. If you are a recipient of this notice, your vehicle is included in this program.
What is the problem?
On certain Mazda6 vehicles, it is possible a crack may occur in the soldered part of the Integrated Circuit (IC) terminal in the Fan Control Module, causing poor continuity. Under this condition, the cooling fan may stop operating, causing the coolant temperature to rise or a fan noise from the module.
What will Mazda do?
Mazda will replace the Fan Control Module with an improved one free of charge. The repair should take approximately 10 minutes to complete. However, it may take longer depending on the service workload at your Mazda dealership.
If you had your vehicle repaired under the previous campaign SSP 60, 2003-2004 Mazda6 Fan Control Module launched in September 2004, it is still necessary to bring your vehicle in for this campaign. The repair parts used for SSP 60 may not have fully corrected the original issue.
As a reminder, the Mazda Driver's Assurance Plan may provide alternate transportation when your vehicle is at an authorized Mazda dealership for a warranty repair. To be eligible for alternate transportation, your vehicle must be a 2001 or newer model and within the mileage and time limitations under the Mazda New Vehicle Limited Warranty and adhere to the Rental Car Reimbursement policy. Ask your dealer for details about the Mazda Rental Car Reimbursement Program.
What should you do?
Mazda is concerned about your complete satisfaction, and we encourage you to make an appointment with any authorized Mazda dealer to have the Fan Control Module inspected, and if necessary to have the unit replaced. You do not need to bring this notice to the dealer, but it may assist in the check-in process.
What if you have already paid for inspection or repair on the Fan Control Module?
If you have already paid for the inspection/replacement of the Fan Control Module due to a problem, you may be eligible for reimbursement of reasonable repair expenses based on Mazda's repair standards. Please complete the enclosed "Reimbursement Application Form," including the necessary documentation, and mail it to us in the pre-addressed envelope provided, allowing 6-8 weeks for processing.

If you are a vehicle lessor receiving this notice, please take steps to ensure that this notice is forwarded to the lessee.
