- With the engine running.
- Minimum MAP reading is 15 kPa (4.4 in hg).
- Electric Vacuum Pump (EVP) is commanded on.

- The EVP is operating and the system is not able to increase vacuum at the rate of 1% per second between the thresholds of 69% and 73% of BARO. The system must accumulate 5 fail attempts to set a fault. These fail attempts can be decremented over time by meeting normal operational requirements.
- The EVP is operating and the system is not able to increase vacuum at the rate of 1% per second when vacuum is under the 69% BARO threshold. A single failed attempt under 69% BARO will result in the P258B DTC.

- Vacuum hose assembly leaking or faulty check valve.
- Brake booster internal leakage.
- Faulty vacuum pressure sensor.
- Loss of battery voltage or ground to the EVP.
- Weak or faulty electric vacuum pump.
